Posted by Alex Espinoza on November 30, 2011 – 12:49 PM

The San Francisco 49ers will say “thanks” to the fans for their tremendous support and will also promote youth wellness via the NFL’s Play 60 program during the last Sunday home game of the 2011 regular season.

The 49ers host NFL West Division rival St. Louis on Sunday at 1:15 p.m. The game has been designated as “Fan Appreciation” day and will be presented by UnitedHeathcare. All fans in attendance receive a “Forever Faithful” scarf. Five lucky fans will have a chance to Punt, Pass and Kick their way toward a $50,000 cash prize. The P,P &K winner will receive a prize for his or her entire row in the stands, an autographed jersey, Una Mas coupons and the opportunity to kick a 30-yard field goal to win $50,000.

There are a limited number of tickets remaining for the St. Louis game. The final home game of the season is against the Pittsburgh Steelers on Monday, Dec. 19. There are no tickets remaining for the Pittsburgh game.

In addition to supporting children’s health, the 49ers Faithful are urged to participate in a holiday toy drive. Donations of new unwrapped toys may be delivered to stadium gates at Candlestick Park.
